In our last post we discussed
about the importance of webinars as they play an important role
from a learning perspective. Our last webinar was on contraception + UKMEC
Criteria which was one of the important topics from
the syllabus of MRCOG examination given
by the Royal College of Obstetricians and Gynaecologists (RCOG).
Recently, we Team CrackingMRCOG
conducted a webinar on “Pre-Invasive Cervical Lesions and Cervical Cancer”. Cervical cancer
is the 4th most common cancer among women and causes more deaths
than any other cancer. There are a number of risk factors for this disease
amongst them; HPV is the most common cause. HPV is found in around 95% of cases
with cervical cancer and most common subtypes include HPV-16 and HPV-18, as
well as -31, -33 and -35.
Early screening of cervical cancer is quite helpful in the detection,
prevention and treatment of the disease. Various screening methods that have
been employed for the diagnosis of cervical cancer which includes:
1.
Cytological screening test:
includes traditional Pap smear and thin layer preparation.
2.
Visual inspection methods:
includes colposcopy.
3.
HPV-DNA genotyping.
4.
Tissue biopsy
5.
Test for staging
Staging of cervical cancer is based on the classification devised by FIGO
and it is mainly of the following types:
- Stage 0
- Stage I
- Stage II
- Stage III
- Stage IV
The treatment of choice is based upon the staging of cancer. Various
management techniques for cervical cancer include:
1.
Surgery
2.
Chemoradiation
3.
Radiation therapy
4.
Chemotherapy
5.
Pelvic exenteration.
The 5 year survival rate for each
stage is different; Stage I (79%), Stage II (47%), Stage III (22%) and Stage IV
(7%). Therefore, early screening and
management will help prevent the deaths of women suffering from this disease.
